【Request for Cooperation】

Based on emergency declaration by the infection spread of the new coronavirus having been given, we have a rotating schedule with regular attendance and telecommuting.

Therefore, it may take longer than usual to issue certificates, so please allow plenty of time for your application.

 

Those who have graduated from the School of Veterinary Medicine or who have completed the Graduate School of Veterinary Medicine and wish to apply for the issuance of certificates of graduation/completion, or for their academic transcript, are asked to complete the following procedures.

 

For enrolled students

 

Application procedures

* We do not accept applications by phone, fax or email.

* We do not issue them on the same day.

* Certificates will be issued as soon as required documents are prepared.

 

 

Application at the academic affairs section

Download the application form for issuance of certificates from the Hokkaido University website, or use one at the academic affairs section of the School of Veterinary Medicine. Fill it out completely and submit it to the academic affairs section.

Certificates cannot be issued on the day the request is submitted. You can pick them up at the academic affairs counter from noon next day or receive them by post. If you wish for them to be delivered, bring stamped, self-addressed envelope.

If you wish for certificates to be delivered, send the following documents to us by post.

Please make sure to apply well in advance, taking into account factors such as postal conditions.

In the event that you cannot print out the application form, write the following items on letter paper.

  1. Name (If you wish to apply for an English-language certificate, write your name in Japanese and Roman characters.)
  2. Birthdate
  3. Student ID number (if you remember it)
  4. Enrollment affiliation (the graduate school department you attended)
  5. Year/month of graduation/completion (or year/month of withdrawal)
  6. Type of certificate required and number of copies (Japanese-language version/English-language version/both)
  7. Place of submission and purpose of use
  8. Address and telephone number of the applicant (the student graduating or completing School/Graduate School curriculum) (a daytime phone number)

* Use the prescribed form when applying for a certificate of degree conferment (doctorate degree holders only).

3. Stamped, self-addressed envelope (with your postal code, name and address, a stamp)

 

● Envelope size

Total weight of 50 g or under (including a reply envelope): 120 mm × 235 mm (an envelope that can hold an A4 sheet of paper folded in thirds)

Total weight of more than 50 g (including a reply envelope): 240 mm × 332 mm (an envelope that can hold an A4 sheet of paper)

 

● Postal rates

* Since the number of pages varies depending on the number of courses enrolled, some documents may differ in weight from those listed above.

* Other certificates without being sealed: about 5 g per certificate

* Postal rates: Please confirm on the Japan Post website the postal rate for mail with the weight above including a reply envelope.

* If the rate is uncertain, send us more postage than is estimated to be required.
Unused stamps will be mailed back with the certificate, but please be aware that we cannot return change because stamps cannot be broken.
(E.g.: The shortfall in postage was 20 yen, and a 50-yen stamp was sent to us. We cannot return the difference of 30 yen.)

  

Applying and receiving from overseas

If you live overseas and need a certificate, ask a friend / acquaintance / somebody of a labo you used to belong to in Japan to request it on your behalf to fill out the application form and submit it for you (paper form, not data).

Please discuss the return envelope and cost of stamps with your proxy. 

The following two items are required in addition to the documents required for the application.
-Letter of attorney : The individual names of the proxy and you are specified.
-Identification of the proxy (photocopy of her/his passport)
If you have no choice but to make a request from overseas by yourself, please enclose an international reply coupon or purchase stamps from the Japan Post website and designate the mailing address as the "Certificate Issue Contact".

 

Number of days required for issuance (after an application form for the issuance of certificates is received)

Japanese-language certificate:

About 1 week after receiving the necessary documents(Due to the effects of the new coronavirus)

English-language certificate:

About 2 weeks after receiving the necessary documents(Due to the effects of the new coronavirus)

* The above guidelines do not include the number of days for mailing. Please note that we are unable to issue in a shorter period of time than this.

 

Notes

  1. No charge is needed for the issuance service. Pay postal fees only when mailing required.
  2. We cannot pay postal fees temporarily on behalf of an applicant. Those who are living overseas and wish to apply for the issuance of certificates should have a Japanese representative to get Japanese stamps ready. (No cash-on-delivery is accepted.)
  3. We do not issue certificates on Saturdays, Sundays, holidays, summer and new year holidays. 

* The information collected at the times of application is appropriately managed and used only for certificate issuance.
